SpamLookup is working great for me.
Not only is it blocking a bunch of spam, but I cleverly added a filter to block all spam with  in it, which is the beginning of an encoded spam message. That seems to be blocking tons of spam, since my real commenters don’t use HTML-encoded entities. Between its general lookup, and that filter, only a couple of things have slipped through, enough for me to manage by hand.
